Blog Post #208 – Furfari Paving Co. Ltd. Fined $110,000 after Worker Killed

Furfari Paving Co. Ltd., a Toronto-based company, was fined $110,000 on July 13, 2010, for a violation of the Occupational Health and Safety Act after a worker was killed.
On July 30, 2008, two workers employed by Furfari were operating an asphalt road milling machine, otherwise known as a “grinder,” to remove the top layer of asphalt from a section of Dixie Rd. between Eglinton Ave. and Matheson Blvd. in Mississauga. Other machines were in use nearby, including a pavement cutter operated by a third worker, who was cutting and sealing cracks in the wake of the grinder. The two workers finished removing the top layer of asphalt and operated the grinder in reverse. When they stopped, they saw that the third worker had been caught underneath the grinder and crushed.

NATSCO Mechanical Contractors Inc., a Sarnia-based company, was fined $70,000 on May 10, 2010 for a violation under the Occupational Health and Safety Act (OHSA) after a worker was struck by a 3,500 kilogram pipe and seriously injured.

Blog Post #174 – Reid & Deleye Contractors Ltd., K & S Forming Inc. and Supervisor Fined a Total Of $104,000 After Worker Injured

Reid & Deleye Contractors Ltd. of Courtland was fined $50,000 on November 27, 2009, for a violation under the Occupational Health and Safety Act (OHSA) after a worker was injured. K & S Forming Inc., a London-based concrete forming company, was fined a total of $50,000 in the same incident and one of its supervisors was also fined $4,000 for violations under the OHSA.

Blog Post #45 – Atlas Dewatering Corporation Fined
Atlas Dewatering Corporation, a Concord company dealing in groundwater control, was fined $75,000 on February 10 for a violation of the Occupational Health and Safety Act after a worker was critically injured. Patrick Maher, a supervisor with the company, was fined $6,000 in relation to the same incident.
On June 12, 2009, Atlas Dewatering Corporation was working at the bridge between Chapman St. and Silver Lake Rd. in Port Dover. The company was installing a temporary cofferdam so that engineers could inspect the existing dam. A cofferdam is an enclosure within a water environment that allows water to be pumped out to create a dry environment. Workers were using a crane to install the cofferdam while an electrical conductor remained energized overhead. A worker came into contact with the crane’s hook and received an electrical shock that caused extensive nerve damage leading to amputation.
