The Corporation of the City of Orillia was fined $50,000 on September 29, 2009, for a violation of the Occupational Health and Safety Act (OHSA) after a worker was injured.

On April 22, 2008, a lighting and sound technician at the Orillia Opera House was trying to descend from a raised audiovisual booth into a change room by way of a hatchway and ladder. On the way through the hatchway, the worker grabbed a hand-hold on the wall. The hand-hold detached from the wall and the worker fell backwards into the change room about 3.6 metres below.

Xerium Canada Inc. Fined $60,000 After Worker Injured

Xerium Canada Inc., a supplier and repairer of the rolls used in paper-making machines, was fined $60,000 on October 2, 2009, for a violation of the Occupational Health and Safety Act (OHSA) after a worker was injured.

On March 19, 2008, workers at Xerium’s North Bay facility were attempting to repair a machine used to mix bonding liquid. The power to the machine was turned off and the protective guards covering the machine’s driveshaft were removed. The workers turned on the machine again, but did not replace the guards right away; the sleeve of one of the workers was caught in its moving parts.

A Brantford construction firm, was fined $250,000 on September 29, 2009, for a violation of the Occupational Health and Safety Act (OHSA) after a worker was killed.

On October 3, 2007, the company was providing general site services for the construction of an electrical plant, in Toronto, for SNC-Lavalin Power Ontario Inc. While installing temporary lighting, an apprentice electrician opened a 600-volt electrical panel and contacted the taps of the live transformer. The worker was electrocuted.

Jackson Builders Supply Limited Fined $55,000 After Worker Critically Injured

Jackson Builders Supply Limited, an Iroquois manufacturer of wood pallets and containers, was fined $55,000 on September 28, 2009, for a violation of the Occupational Health and Safety Act (OHSA) after a worker was critically injured.

On February 5, 2008, two workers were on the facility’s recycling line, examining pallets for damage and making repairs. They were standing in the output area of a pallet turner, a hydraulically driven machine that turns over pallets for further repair. A chain on the pallet turner came loose, and the machine was shut down with a pallet left inside. This pallet had a hammer on it. When the chain was put back on, and the machine restarted, the pallet inside was rotated and the hammer fell on the floor. One worker told the other about the fallen hammer. The second worker, erroneously believing that the first worker had activated the machine’s emergency stop, reached between its steel tubing and its rotating leg to retrieve the hammer. The machine detected an incoming pallet and cycled, causing the worker’s arm to be crushed between the leg and frame of the pallet turner.

Liquidation World Fined $18,000 For Health and Safety Violation

Liquidation World Inc., an Alberta discount retailer, was fined $18,000 today for a violation of the Occupational Health and Safety Act (OHSA).

On September 23, 2008, an inspector visited Liquidation World’s Brantford location as part of the Ministry of Labour’s safety blitz on falls in industrial workplaces. The inspector found that two workers had been using an unsafe ladder. The ladder was missing three of its four non-slip feet, and one of its cross-members was broken.
