Four construction workers at a sports complex that will serve as the practice facility for the Montreal Canadiens escaped with only minor injuries after a portion of the structure collapsed on February 25.

The $30-million complex, being developed by Axor Inc., is to be located in the southern Montreal suburb of Brossard and is expected to be completed this fall. It will house two ice rinks, an indoor soccer field and other training facilities.

Sousa Ready Mix (1011786 Ontario Inc.) of Glenburnie was fined $80,000 on October 22, 2009, for a violation under the Occupational Health and Safety Act (OHSA) after a worker was injured.

On October 22, 2007, a Sousa worker was trying to clear sand from a conveyor while it was in motion. The worker’s arm got caught in the conveyor and was seriously injured.

Blog Post #156 – Kingdom Construction Limited Fined $105,000 After Worker Killed

Kingdom Construction Limited was fined $105,000 on October 19, 2009, for a violation of the Occupational Health and Safety Act (OHSA) after a worker was killed.

On April 22, 2008, Kingdom Construction was building a sewage-pumping station in Halton Hills. At the location there was a wet well, a concrete chamber used to collect sewage until it is pumped to a treatment centre. The top of the wet well was above ground with two openings. When not in use, one opening was covered with plywood. Workers were tidying the top of the wet well when one of the workers fell through that opening and was killed.

Dashwood Industries Limited, a window and door manufacturer, was fined $65,000 on October 19, 2009, for a violation of the Occupational Health and Safety Act (OHSA) after a worker was injured.

On September 15, 2008, a worker at Dashwood’s facility in Centralia was dismantling the roof of a sound enclosure. The sound enclosure had been used to mute the sound from a line of milling machines that were no longer in use. The worker, on the roof of the enclosure, unhooked from the lifeline and moved to an access ladder to get down. The worker stepped on a plywood plank in front of the access ladder, and the plywood, which was not secured in place, slipped off the roof. The worker fell about four metres to the ground below, sustaining wrist and head injuries.
